Karibu care Services provides:

Individualized Home Supports

It is aimed at individuals who require support with daily living activities, such as personal care, household chores, and social engagement, but prefer to remain in the comfort of their own homes rather than transitioning to a care facility.

Respite Care

Karibu care is a service that provides temporary relief and support to caregivers who are responsible for the care of individuals with disabilities, chronic illnesses, or special needs.

Homemaker

Karibu care services can provide support with household tasks and chores for individuals who require assistance due to health-related challenges.

Night Supervision Services

Night supervision services refer to a type of support provided during overnight hours to individuals who require assistance, monitoring, or supervision during that time.

24-hour emergency assistance

24-hour emergency assistance is a service designed to provide immediate help and support in times of urgent situations or emergencies. This service does not replace the use of 911 when necessary.

Integrated Community Supports (ICS

Integrated community supports (ICS): Services that provide support and training/habilitation in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building).

Individualized Home Supports

The primary objective of Individualized Home Supports is to enhance the individual’s quality of life and promote independence by providing tailored assistance and support. The service is typically delivered by trained professionals or caregivers who have experience in working with individuals with diverse needs.

Individualized home supports cover community living services in four categories:

Community Participation

  • Community mobility and pedestrian safety (e.g., safely getting in and around the community)Community mobility and pedestrian safety (e.g., safely getting in and around the community)
  • Community safety and awareness
  • Informal support system and network development
  • Interpersonal communications skills
  • Leisure, recreation and socialization planning
  • Skill-building to meet transportation need

Health, Safety, and Wellness

  • Collaboration with the person to arrange health care meaningful activities, social services, meetings and appointments.
  • Cueing, guidance, supervision, training or instructional support to complete self-cares (Note: Cannot duplicate use of eligible Medical Assistance [MA] state plan home care services; see CBSM – Home care overview)
  • Health services support, as defined in Minn. Stat. §245D.05
  • Help for the person to activate and build resiliency factors.
  • Support for the person to design and meet individualized strategies to reach their health, safety and wellness goals.

Household Management

  • Cueing, guidance, supervision, training, or instructional support to complete routine household cares and maintenance
  • Household safety knowledge and skills
  • Tenancy support and advocacy
  • Budgeting and assistance to manage
  • Cooking, meal planning and nutrition
  • Healthy lifestyle skills and practices
  • Household chores, including minor household maintenance activities
  • Personal needs purchasing

Adaptive Skills

  • Crisis prevention skills
  • Implementation of positive support strategies
  • Problem-solving
  • Sensory/motor development involved in acquiring functional skills
  • Support strategies for self-sufficiency
  • Support and training to increase positive behavior resulting in the reduction or elimination of maladaptive behavior

Respite Care

Respite care is a service designed to provide temporary relief and support to primary caregivers who are responsible for the care of individuals with disabilities, chronic illnesses, or special needs. It offers caregivers a break from their caregiving duties, allowing them to rest, recharge, or attend to their own personal needs while ensuring that their loved ones continue to receive necessary care and supervision.

A person is eligible to receive respite if the primary caregiver is absent or needs relief from his/her caregiver duties. See the following sections for additional program-specific primary caregiver requirements

AC and EW

  • Not to be paid at all.
  • Be paid only for a portion of the time he/she provides care/supervision to the person.

BI, CAC, CADI and DD

  • Maintain his/her primary residence at the same address as the person
  • Be the owner or leaser of the primary residence

Homemaker

Services that help a person manage general cleaning and household activities.

Eligibility: A person is eligible to receive homemaker services when either of the following is true.
The person needs support to manage the general cleaning and household activities
The primary caregiver who is regularly responsible for these activities is unable to manage them or is temporarily absent. Refer to the secondary information section for additional information about primary caregivers.

Housekeeping Services

  • Washing dishes
  • Sweeping, mopping, vacuuming
  • Light dusting
  • Changing linens
  • Taking out trash
  • Laundry

Night Supervision Services

Night supervision services: Overnight assistance and supervision provided by a staff in the person’s own home when the person has an assessed need in one of the covered services areas.

Night supervision services refer to a type of support provided during overnight hours to individuals who require assistance, monitoring, or supervision during that time. These services are typically designed for individuals who may have specific needs or conditions that require additional attention or care during the nighttime.

Night supervision services

  • Implementation of the person’s positive support programming and transition plans.
  • Reinforcement of skill development supports
  • Assistance with activities of daily living(ADLs).
  • Assistance with instrumental activities of daily living (IADLs).
  • Limitations: A person cannot receive night supervision services for more than 12 hours in a 24-hour period.

24-hour emergency assistance

On-call counseling, on-call problem solving and/or immediate response for assistance at a person’s home due to a health or personal emergency that is not related to an injury or an immediate danger. This service does not replace the use of 911 when necessary.

24-hour emergency assistance

  • Eligibility: A person is eligible to receive 24-hour emergency assistance if they meet one of the following sets of criteria:
  • Lives in their own home
  • Does not receive 24-hour supervision
  • Would otherwise require extensive, routine supervision in the absence of this service.

Integrated Community Supports (ICS)

Integrated community supports (ICS): Services that provide support and training/habilitation in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person’s living unit or in the community.

Integrated Community Supports (ICS)

ICS covers training/habilitation and support to meet the person’s individualized assessed needs and goals in at least one of the community living service categories:

  • Community participation.
  • Health, safety and wellness.
  • Household management.
  • Adaptive skills.
